An internal Dublin Fire Brigade report recommended the serving of a fire notice on Longboat Quay in May of last year.
It comes as an emergency motion on the complex will be discussed by Dublin City Council this evening.
According to the Irish Examiner, if the fire notice had been served last year, it would almost certainly have resulted in the evacuation of the Docklands apartment block.
The recommendation came from a senior executive who found 12 separate areas of concern.
They include the lobby at each floor not provided with adequate smoke vents, and common areas not properly sealed or fire stopped.
Dublin Fire Brigade issued a fire service notice last week, which will require repairs to be carried out by a specific date at a cost of 4 million euro.
