An investigation is underway after a baby died at Holles Street Hospital.
The Health Service Executive has confirmed that an investigation is underway following the death of a baby.
The baby was born on the 16th of August at the Midland Regional Hospital in Mullingar, but subsequently died at the National Maternity Hospital Neo Natal Unit on the 22nd of August.
In a statement, the HSE says an "incident management policy is being followed" and a "full and thorough investigation has commenced".
The executive says the family are involved in the process.
The Hospital has extended its sympathies to the family.
