The Children's Ombusdman says two Roma children taken into care last year shouldn't have been.
They were removed from their families after concerns were raised with Gardai about their identities.
Emily Logan says the families were "humiliated and embarrassed" as a result, and that the incidents in Tallaght and Athlone shouldn’t have happened.
The report said in the case of the young girl from Tallaght, she was taken because of a number of issues including inaccurate information from the Coombe Hospital and their readiness to believe she was abducted because she had blonde hair and blue eyes.
While the Ombudsman said the Athlone case should have been put to rest when the childs father and public health nurse told Gardai that the child has Albinism.
The Justice Minister has met with both families to personally apologise, while Enda Kenny has publically apologised in the Dail.
The 7 year old girl in Tallaght has since dyed her hair black, because she fears that she may be taken from her parents again.
