A damning report into the death of a Dublin teen in state care reveals gaps in the system.

It's one of 5 cases to be published today.

Danny Talbot was 19 when he overdosed in State care in 2009.

He first came to the attention of social services as a toddler but a series of missed opportunities contributed to his premature death.

The report by the Child and Family agency and in the Irish Times, highlights gaps in mental health services, poor frontline social work practices and a failure to recognised evidence of abuse and neglect.

It's one of 5 cases of deaths or serious incidents involving children and comes at a time of rising concerns over the level of funding available.

There are currently about 200 vacancies in social work teams around the country,
